About Care Management
Having a care management plan means taking the stress out of your care. If you’re juggling multiple service providers or appointments, we can simplify everything for you into one point of contact.
All Dovida clients have a dedicated Care Manager who will help you plan, navigate, co-ordinate and manage your care needs and services. As your needs change over time, your Care Manager will be right there with you, making sure you are consulted and informed at every step.
Care Managers are experienced, thoughtful, and understand the services available in your local area. They know how things work, how much they should cost, and which services will be most beneficial for you.

How Care Management Works
You’ll be assigned a designated Care Plan Manager, who will take over the administration and organisation of your care, so you don’t have to worry about it.
Your Care Plan Manager will organise your appointments, services, home modifications and equipment, and anything else you need for your care to run smoothly.

Firstly, contact our team who can work with you to understand your individual needs. A care plan will then be created and your Care Manager will then work closely with you to start organising your care in a way that works for you. This will include creating a visit schedule, matching caregivers and organising additional appointments like allied health visits.

What’s the difference between care management and home care?
Home care is the services that come to your home to help and care for you, whereas care management means that somebody will oversee and organise this care and any other care you need including creation of a care plan.
